When coming to the office for the first time, you will complete paperwork that gives us detailed information about your current health, current symptoms, current medications, past medical history, past surgical history and family medical history. We recommend bringing a list of medications, doctor’s notes, lab work, tests and any documents that will help us better obtain this information. You will also need a picture ID, insurance cards and copay. To make this initial visit a little quicker, we can email or fax the paperwork so it can be completed prior to arrival. When completing this paperwork, please be thorough and honest to ensure that we provide you with the best care possible.

What to Expect

When you arrive at the office, you will be greeted by our friendly front office staff. The front office staff will verify your information upon arrival. Once check-in process is complete, you will be taken to the exam area where our medical team of trained nurses and medical assistants will answer any questions you may have. The physicians take pride in providing the highest level of care to each patient. Most surgeries will be set up for a later date.

Our team of physicians have specialties in Internal Medicine, Podiatry, and Surgery. Dr. Carla Thurston currently services patients for Internal Medicine at our Morgan City location. Dr. Adam Ziegenbusch and Dr. Andrea Leonards are currently seeing patients in Houma and Morgan City for Podiatry. Dr. Chris Berry and Dr. Neil Lyons are our team of surgeons who can perform surgery at Teche Regional Medical Center, Terrebonne General Medical Center and Physicians Medical Center. Furthermore, we accept most insurance plans, self-pay, and have payment plans available. You should expect to receive compassionate, quality care from SLMA.
